Requesting a U of G Qualtrics Account

Self-enrollment for Qualtrics accounts is currently unavailable.

To request a Qualtrics account, U of G faculty, staff and students can submit a request for a Qualtrics account by completing the Working With Data Request form.

  1. Under "What can we help you with?" choose "Other"
  2. In the "Please tell us a bit more about how we can help you" field type "Need Qualtrics account"
  3. Complete the rest of the form, ensuring that you provide your correct name and email.

Once the form has been submitted library staff will respond as soon as they are able and provide you with instructions for accessing your Qualtrics account. Thanks for your patience.
